Every volunteer working with children, youth, or persons with disabilities needs to have a background check run, preferably before beginning service.  Each volunteer also needs to complete a Volunteer Profile Packet.

In an effort to make this easier for you, these documents are available below, which may be completed on the computer and sent to us via email or you may print them out and complete them hand if you desire (Please use the printable forms in this case.)

Please don't complete these forms before talking first with someone from the ministry with which you want to serve.

If you are a NEW volunteer:

Please read the protection policy and complete these forms. Due to the information provided in the forms, we would prefer that you deliver a hard copy to the church office.
